• Draft of the instrument was developed and field tested with 6 programs. Seven participants from each program responded (1 program administrator, 2 service providers, 2 family members, 2 consumers).
• A short questionnaire was administered to all participants to obtain their feedback and general impressions of the instrument and the process. Package of instruments was revised on the basis of the feedback in preparation for the pilot-testing phase.
